Over 75 Health Assessments

Each year Pivotal Health invites our over 75 year old patients to attend the practice to have their Health Assessment completed. This is a bulk billed service.

What is an over 75 Health Assessment?

An Over 75 Health Assessment is a comprehensive assessment of your health and your physical, psychological and social function. It helps to assist in the early identification of care needs.

WHO IS ELIGIBLE TO RECEIVE A HEALTH ASSESSMENT?

You are eligible if you are aged 75 years or over and living in a community setting. You must be a regular patient of the surgery. Patients are only eligible to have one health assessment completed each year.

WHAT HAPPENS DURING A HEALTH ASSESSMENT?

You will spend between 20-40 minutes with the Practice Nurse who will perform some simple tests and ask you some questions about your medical history.

This might include the following:

  • Vision check
  • Hearing check
  • Skin check
  • Urine sample
  • Blood Pressure
  • Medication review
  • Immunisation status
  • Weight
  • Height
  • Questions about your nutrition/diet
  • Questions about your social activities
  • Questions about your daily activities

A detailed record of this assessment with the outcomes and recommendations is recorded in the patient file and the patient is given a copy to keep.
