Repeat Prescription Request
The Repeat Prescription Policy & Procedure should be read before completing this form.
Please note that medications that are unable to be prescribed using this service include pain medications, weight loss medications, benzodiazapines (eg Valium) and stimulant medications (ADHD).
On receipt of the request a Pivotal Health team member to ensure that the request meets the criteria set by the practice and the request will then be forwarded to your usual GP. In the event that the request does not meet the criteria you will be contacted and asked to make a face to face appointment.
We ask that you allow at least 3 business days for this request to be completed. In the event that the prescription is needed urgently it is at the doctor’s discretion whether a surcharge fee applies.
